My Premise Health is a secure online and mobile tool that connects you with your healthcare experience by providing convenient virtual visits, access to your providers, health records, visit history, test results, and more

Spouse and Dependent Sign-up:

  1. Complete registration at the time of your next appointment; or

  2. Email and provide a phone number
    for a return call. Available Monday – Friday, 6 a.m. – 7 p.m. CST; or

  3. Visit or call the Center to receive a personalized registration invitation via email or text.

  4. Once you have created an account online, you can download and log
    in to the mobile app.

Employee Sign-up:

  1. Visit or download the mobile app
    from the App Store or Get it on Google Play

  2. Click on “SIGN UP NOW”

  3. You can self-sign up (using your legal name) by verifying the last
    four digits of your social security number (SSN) and date of birth.
